I lead engineering on a clinical documentation platform, and most of the difficulty sits in the multi-agent architecture: retrieval across 500,000+ documents from sources a clinician will accept, with every answer anchored to a citation they can follow back. We also automated WHO ICD-11 coding with post-coordination, alongside SNOMED — 200,000+ codes and effectively unlimited combinations, derived from live patient-doctor conversation. I have not seen anyone else ship that. The move off Azure onto GCP ran in parallel, without downtime.
